Appointments

If you would like to schedule a visit at our office, please call us during our business hours. You can also request an appointment online by selecting a provider on our care team or through your MyNovant account after you've had at least one appointment. If payment is a concern for you, please mention this when you schedule your appointment so we can assist you.

When you visit, please bring:

• Your photo ID, if you own one
• Your insurance card, if you are covered by insurance
• A method of payment to cover the cost of your visit
• Forms you need your provider to complete, including sports or school physical forms
• A list of the medications you are currently taking, including supplements
• A list of the vitamins and medications your child is taking, if your child is the patient
